So far it has been a really good experience. Some of the CNAs are too young, many are amazing and really care. They treat me great. Physical therapy is great but I would like more exercises. The food is good so far. The facility is clean and comfortable. Overall a really great experience.
My mother was at Lakeport Post Acute for 100 days, released home today! We were admitted to a different facility at first and I was so grateful that we were able to be moved much closer to home. From the moment we were accepted there, everyone from administration to transport to dietician to cleaning staff were professional and kind. The therapy staff was so incredibly kind and conscientious. And they worked hard to get her up and going! There was a time when she was first admitted we weren’t sure she would be coming home, but they worked like crazy to get her mobile again. All the nursing staff, from RNs to CNAs to everyone in between was thoughtful and lovely. Truly, my family doubted she would make such an incredible recovery. We credit that to how conscientious the therapists were and the care of all who cared for her. What you do isn’t easy, yet you do it with grace and kindness. I cannot thank the whole team enough!
They helped me recover from pneumonia and regain my strength. I love the staff, physical therapy and activities.
I recently stayed at Lakeport post acute. I found the facility to be fantastic, the staff was very helpful and attentive, the food was good, dietary staff was very helpful. The facility was very clean. Houskeepg was fantastic. Therapy was top notch. I would recommend wholeheartedly coming to Lakeport Post Acute if you ever need the care.
I’m very happy with the care I am receiving here. The staff is very friendly and responsive. The OT and PT teams are great. They really know their stuff. They have been very helpful. The staff have a good relationship with each other and constantly help each other out. Everyone is very cooperative. It makes for a very supportive climate in which to heal.
My 82 year old aunt stayed at Lakeport Post Acute for a month and the staff was excellent! Everyone was very attentive and friendly. The facility was bright and clean with lots to keep her busy.. she loved the staff members that would stop for small conversation and to listen to her jokes. The physical therapy there was a lifesaver! The PTs were so caring and helpful and really made the difference in her recovery! She was able to receive her instacart orders easily but she enjoyed the in-house meals often asking for seconds. I highly recommend Lakeport Post Acute to anyone looking for exceptional aftercare!
I recently spent three months at this facility after a nasty fracture of my left leg. The physical therapy department is amazing! Experience, knowledge and empathy! First time attempting to put a little weight on the leg I cried! With patience and kindness, I was able to learn the best way to function without hurting myself. Very grateful. Overall care was satisfactory.
My heartfelt thanks go out to the entire staff at Post-Acute Rehab for their incredible care and support during my two-month recovery from a broken hip. I can’t say enough about the kindness and professionalism of everyone there. The nurses and CNAs were absolutely wonderful, the food was surprisingly great, and the physical and occupational therapy team—especially Shobha, Keysap, Stuart, Jesus, and their whole crew—were phenomenal. They treated me with such compassion, patience, and encouragement every step of the way. I felt truly cared for, like part of the family. I’m so grateful to each of them and would highly recommend this facility to anyone in need of skilled, heart-centered recovery care.
We had an amazing experience with the physical therapy team at the post-acute rehab facility. They were compassionate, respectful, and treated my mom like family throughout her recovery. Their dedication and care made a huge difference in her healing journey. A special shout-out to Shobha and Keysap—thank you for going above and beyond! I would highly recommend this team to anyone in need of physical therapy.

Lakeport post Acute is a very good facility. Very clean, the CNA’s work hard to give every resident very personalized attention, your loved one’s will be treated with dignity and respect here.
I recently spent almost a month at Lakeport Post Acute Rehab. It was, from the beginning, being greeted by Brian, the Intake Administrator & 2 very friendly & helpful CNA’s, an excellent experience. Everyone was so helpful & caring & even had a great sense of humor. Just wanted to thank the entire staff & management for their empathy & caring. Sincerely, Jan Bell
Clean facility! Responsive staff that demonstrate professionalism. Residents appear clean and relaxed. No games regarding bed availability- short wait period versus other places that gave us the runaround. Optimistic.

My 90 year old father came here for comfort (hospice) care two weeks ago. Literally everyone there were kind, helpful and it was a good feeling knowing they were looking after him. Even the food looked quite good. It is stressful watching your father die. Their treatment of him relieved much stress from us. I’m very thankful he was able to be here.

I absolutely love this place the staff is so compassionate and caring from the admissions director to the dietary manager listened and worked hard to make this the easiest it could be considering everything we had been through. I would definitely say if you want to have a good experience in a rough time I would say Lakeport Post Acute is where I want to go ..

I’m beyond disgusted with the treatment I have received in regards to a girlfriend of mine that was just discharged from there. I am after doing some paper reading and some investigating of my own dumbfounded at the fact that she was put in there because of hypertensive heart disease and in actuality the problem was she has a slipped hardware in a knee that she had surgery on years ago and it’s keeping her from being able to walk while being there she hurt her shoulder exercising on top of the knee problem when she needed surgery on her knee first not physical therapy she hurt her arm while doing exercises so now her right knee and her left shoulder is out I can’t barely move her she’s having a hard time and I’ve called various times throughout the week to ask different questions I always have to leave a message and I always get no call back it’s very disheartening when you’re doing the best you can to care for somebody and you can’t even get them to return a phone call thank you Sarah I waited all day to hear back from you and nothing called three times I spoke to you in person once and was told you to get back to me at 10:30 or 11:00 it’s now 3:00 and I’ve called three times having to leave a message that is blatant disregard for somebody I have to care for and I don’t appreciate it at all. Seems maybe I will start writing letters and emails and see if that helps. It’s imperative that they send these patients home with the right amount of medication that they need until they see a PCP which in fact is the scheduling that they did at the facility and she’s almost out of her medication and we don’t get to see her PCP for two more weeks I just don’t even know what to do I’m disgusted.
The good things: My mom recently spent about a month there after breaking her hip in a fall. From the time she entered, until now, she has improved immensely. Their food is excellent, healthy and nicely prepared. The bad things: they give each patient a red button that is attached to bed. Obviously, you ring it if you need assistance. It’s a joke, they take between 1-2 hours to respond. They don’t even walk by your room to see if you are okay. The care is inconsistent, depending what CNA you get. The nurses are different at least three times a week. The communication between employees is almost non-existent, nothing is related from shift to shift. The male receptionist is great, the female doesn’t even look up from her phone when you walk in. Billing department is excellent. Sometimes it took days to catch someone in their office, then they say they have to much to do. There is a general manager who is “there”, but not involved where the real problems are. They need an operations manager, not a manager who is in his office all-day with no clue what is going on in hospital. The van driver was awesome! So there are good parts, and bad parts. My advice, keep a close eye on your loved one while there.
